HMIS Program Request Form

The HMIS Program Request Form is for Technical Administrators (also known as HMIS Agency Leads) to submit HMIS system changes to the Community Administrators. Only Technical Administrators should submit a request form. If you are not sure which staff at your agency is the HMIS Technical Administrator, please reach to the Help Desk at KCSupport@bitfocus.com . 

 

The Community Administrators will review submitted requests with KCRHA. The amount and accuracy of information provided in the submitted request will determine the approval and completion of the request. 

 

This following requests may be submitted using the form: 

  • New Program setup / Program Closure
  • Inventory Adjustments
  • Funding Source Updates
  • Adding Services to a Program
  • Program or Grant Consolidation
  • Program Name Change
  • Program Type Change
